Behavioural Support Referral Form

What is the Project?

This pilot project is the first in the UK to provide behavioural services for people with dementia by Board Certified Behaviour Analysts. The services are being funded by the Regional Collaboration Team for a small number of clients who live in their own homes across North Wales. Behaviour Analysts provide consultation and assistance for a range of behaviours seen in people with dementia, this includes wandering, repetitive question asking, aggression and agitation, and refusal to accept support with personal care.

The goal is to provide support with challenging behaviour to reduce the need for antipsychotic medication and to enable people to continue living independently at home for as long as possible. The pilot began in 2022 and we are currently accepting a small number of clients in all six counties across North Wales.

Who is Eligible for these Services? 

A client who is eligible for this project should:

  • Have a diagnosis of dementia
  • Ideally be living in their own home within the community (we do have some capacity for those living in care settings)
  • Have some form of behavioural issue that is causing problems for their loved ones or carers (examples include but are not limited to: aggression, agitation, repetitive question asking, refusing to accept personal care, wandering, or other challenging behaviours). 
  • This behaviour should happen frequently (At least daily or if less than this it should be easy to predict e.g., every shower time) and we need to be able to observe the behaviour itself multiple times so that we can find out why it is happening (the observations can be in-person or via video).

What Happens if a Client is Chosen for this Project?

If a client is chosen for this project a Board Certified Behaviour Analyst from PACT will come to talk with them, their family, and their carers. We will then complete several assessments and observations of the behaviour to find out why the behaviour is happening and what it means for that person.

Using this information and working together with the family and/or carers, we will develop a personalised plan to change the way the person is supported to change the behaviour or manage it more effectively. We then work closely with the family and/or carers to implement the plan and see whether it works; if it does- great! If it doesn’t, then we will modify the plan and try another approach.

Behavioural services can be very effective to help clients and their carers with various behaviours, however it takes time, effort, and everybody working together for it to work properly. If you are part of this pilot, you can expect a number of home visits from the Behaviour Analyst over a few weeks as a minimum. 

Who can make a referral?

If you care for or work with someone who has dementia and you think they might benefit from our behavioural services and they meet the eligibility criteria above, then please make a referral to us. We take referrals from family and loved ones, social workers, dementia care practitioners, care assistants and third sector workers amongst others. 

What happens next?

After we receive your referral we will be in touch with the person who has dementia or their loved ones/or carers. We will let you know that we have received your referral and discuss with you further.
